Registrar Forms

• Request a Transcript - Use this form to request official SU transcripts. For current and former students.
• FERPA Request to Share Information - This form allows students to determine who SU may talk to about their academic records. We encourage all students to read this form carefully, complete it, and submit it to the registrar's office. Without a completed authorization on file, SU may not speak to anyone but the student about the academic record.
• Change of Name/Address form - Use this form to request a change to your legal name or address. Please note, we will not make these changes without your signature, so read and complete the form carefully.
